WellSky® Technology Empowers Care Coordination Between Area Agencies on Aging and In-Home Care Providers to Improve Outcomes
A powerful new integration for WellSky Aging & Disability solution optimizes referral management and communications with providers to better support seniors and people with disabilities
OVERLAND PARK, Kan. — August 13, 2020 — WellSky, a leading global health and community care technology and services company, today announced a new integration that allows users of WellSky Aging & Disability to connect to personal care providers via ClearCare and WellSky’s interoperable coordination platform. This integration will enhance collaboration between more than 350 Area Agencies on Aging (AAAs) and their provider networks, as well as WellSky’s extensive network of care providers, which includes more than 4,000 personal care agencies using ClearCare. Together, AAAs and home-based care providers can more easily collaborate to deliver essential services to older adults and people with disabilities, while also tracking care referral outcomes.

AAAs provide vital information and services that help people continue living within their homes for as long as possible safely and with dignity. AAAs also manage and coordinate person-centered care for seniors and people with disabilities in their communities. In-home providers include home health agencies, hospice agencies, and personal care (also commonly referred to as “home care” or “private duty”) agencies that support people who need assistance with activities of daily living. With this integration, AAAs can partner even more closely with their provider networks. They can also access WellSky’s network of over 10,000 in-home care agencies and 1 million nurses and caregivers to deliver comprehensive in-home care.

In-home care providers will be able to access and respond to service referrals via a portal powered by the WellSky Care Coordination platform, or directly within ClearCare’s personal care agency platform. The integration allows service providers to participate in the referral system without a license to WellSky Aging & Disability, so any provider can participate in the online referral system.
