WellSky Financial Services LLC Terms

  1. Additional Terms: WellSky Financial Services LLC (“WFS”) is an Affiliate of WellSky.  The following terms are applicable only to the HHCAHPS Services provided by WFS under this Addendum:
    1. CAHPS Services: The CAHPS Services to be provided by WFS under this Addendum include the services enumerated below:
      1. Customized CAHPS Survey and Cover Letter: A survey instrument and cover letter are provided to the Client.  The Client’s logo and up to two (2) additional questions may be added at no additional cost to Client.
      2. Mixed Mode Data Collection: Survey and cover letter are mailed within three (3) weeks of the month end.  Approximately three (3) weeks after the survey is mailed, a telephone follow-up is conducted for those that did not respond to the mailed survey.  WFS performs all data collection functions for both the mailing and telephone calls from printing through data input. -or- Mail Mode Data Collection:  Survey and cover letter are mailed within three (3) weeks of the month end. A second survey is mailed three (3) weeks later to those that did not respond to the first survey. The data collection period is forty-two (42) days/six (6) weeks.
      3. Accurate Data Collection: The sampling and data collection will adhere to all CAHPS Protocols and Guidelines. A Client data file will be submitted to the CAHPS Survey Data Center by the required deadline each quarter.
      4. Patient Satisfaction CAHPS Benchmark Report: The on-demand report offers overall results, trends and compares Clients results against state and national averages. The report will focus on the publicly reported CAHPS measures. WFS’s results are not the official CAHPS results. Visit Home Health Compare for the official, nationally adjusted data.
      5. Unlimited Access to Satisfaction Surveys and Reports: This web based on-demand reporting site offers comprehensive and user friendly benchmark reports, individual survey review and capacity to printout individual surveys. Client will receive passwords to access this protected area. The authorized users must be identified by the Client administrator.
      6. Executive Consultation: Consultation with a Senior Consultant is scheduled at the convenience of the Client to discuss and analyze the information contained in Client’s patient satisfaction reports.
      7. Best Practice Improvement Trainings: The service offers regular sessions per year to all agencies using WFS’s service.
      8. Compliance Newsletter: All WFS Clients with an active subscription to the Products shall receive regular email messages focusing on relevant topics throughout the year.
      9. HIPAA Compliance/Confidentiality: All information received and processed from Client is maintained and/or destroyed using procedures intended to ensure the confidentiality of agency and patient information. WFS will not use or disclose Client’s individual data except in the provision of the service to Client or as authorized by Client. The website and database are encrypted and password protected. Only Client authorized users will be provided access to web reporting.
      10. Training, Technical Assistance, and Support: A welcome packet with orientation materials and a checklist is sent to each new agency to help ensure an efficient implementation according to the CAHPS guidelines. WFS also provides individual and group trainings to the database and reporting system as needed, at no additional cost to the agency. A User’s Guide is available on the reporting web page.
    2. Client Commitments. WFS’s obligations are conditions upon Client fulfilling its obligations hereunder, including, without limitation:
      1. Meet All Data Collection Requirements: The patient file shall be transmitted by Client within seven (7) calendar days after each month end. The patient file must contain all patients eligible to receive the CAHPS survey. The patient file should comply with all of the CAHPS file specifications to ensure accurate and timely data collection.
      2. Comply with all License Provisions, Copyrights, and Confidentiality Standards: Usernames and passwords may not be shared. The survey, reports, and data may not be modified in any way or shared outside of the organization by Client without written authorization. Client will notify WFS if an authorized user to the system requires a change in authorization status.
      3. Exemption Procedure: If Client has filed for exemption status, please submit documentation of such request. Exempt Clients do not have their results sent to the CAHPS Data Center.
      4. Verify Data Submission: Review data submission reports on the CAHPS Coordination Team’s website each quarter.
