WellSky® Partners with Southeastern Michigan Health Association to Further Social Determinants of Health Initiatives
New partnership establishes cooperative mission to improve social care coordination and delivery through cross-sector collaboration
OVERLAND PARK, Kan. — March 24, 2022 — WellSky, a leading health and community care technology company, today announced a partnership with the Southeastern Michigan Health Association (SEMHA) to collaborate on initiatives that target and raise awareness for social determinants of health to achieve better health outcomes in Michigan communities.
A leading public health association in Michigan, SEMHA provides value-based contract management services to state and local health agencies, private foundations, and health programs serving Michigan residents. Working closely with health departments, health plans, health care systems, community agencies, and community-based organizations, SEMHA facilitates collaborative health initiatives through data-sharing capabilities, technological implementations, and operational support.
In partnership with WellSky, SEMHA will widen its network of healthcare and social service organizations, enabling better cross-sector collaboration and information gathering among key community partners.
